My physician seen a pt that has out of state Medicaid. I'm in Florida. I think I would send my claim to my local Medicaid office ? Any input is appreciated.

Hi, Did your office receive a copy of the patient's insurance card? If so, the claims address and or phone number to call will be listed on the back of the card. If no card has been obtained, you will have to contact the patient to get the information that is listed on the card. Hope this helps...
