Patient received Avastin from 10:10-10:40,
Atropine push from 10:35-10:40, Camptosar from 10:44-12:45 and Leucovorin from 10:43-12:45.

I came up with:
96413 = 1
96415 = 2
96417 = 1
96368 = 1
96375 = 1

Please help, I have not done chemo in 7 years and I don't think I am right.

Atropine IVP 10:35-10:40 (5 min) 96375
Camptosar from 10:44-12:45 (2 hrs) 96417, 96415 x 1
