Outpatient facility is normally billed using the UB-04 or the electronic equivalent. The UB-04 billing manual should address most questions. In addition there are numerous transmittals regarding OPPS billing that should be very helpful. Just use keywords UB-04 and OPPS and you will gain a ton of information.
