Hi everybody-
I've asked a couple of people about this question and received different answers so I thought I'd put it up here. I often receive OP orders with both a written dx and a dx code. For instance I received an outpatient wound care order with a written dx of "skin tear right arm" along with the code T81.30XA-Disruption of wound unspecified, initial encounter. Another example - the code on an order will use the "subsequent visit" 7th digit when the wound is still being actively treated. Is it appropriate to change the code to match the written diagnosis or should the provider be queried?
