If a PA does a consultation and discusses the care with the Supervising Physician, can the physician file the charge under their # without going to the patients room? Can subsequent visits be considered "Incident To"?
 
No

No ... physician must have a face-to-face encounter with the patient, must take the history, perform the exam, arrive at an assessment and plan in order to bill under his own name for a consult/new patient.

Bill this under the PA's name/NPI
