PA from a surgical practice doing surgery at the Hospital

The composite board is very vague on what and what not a PA can do from a surgical practice. We have a PA that is doing more and more surgery. He is performing the surgery, documenting himself as the surgeon and saying that the MD was supervising. Now he has ventured into surgery under General Anesthesia. It is my understanding that surgery he is doing needs to be approved by the Composite board? But, how should the op note read. Should he just add himself as the performing provider and then put the attestation that the supervising physician was in the OR for the entire case? Can someone give me some guidance on this. Thank you so much,
