Wiki PA Medicaid 96372 denying for modifier

PA Medicaid is denying 96372 for CO4 Px incons w/mod, req modifier missing. The only other charge for the date is J3420 for Vitamin B12, there are no other services. Is anyone familiar with what type of modifier PA Medicaid is requiring for 96372? Thank you.
 
if billed with E/M same day possibly modifier 25 on E/M level. If billed on same day with another I&I then 96372 will need X sub mod.
