Wiki PA note vs. resident note

Morning. Confused about a physician's assistant note vs. a resident note. If the PA or resident sees a patient initially in the hospital and performs the H&P. Then the physician cosigns the note and charges for the initial hospital visit. Does the physician need to do a note of his own or dictate an addendum agreeing with the PA or resident's note?

For then PA the physician must see the patient face to face on the same day and write a separate note which can include the information from the PA note. As far as the resident, I do believe the provider must be physically present at the time of the visit and must note that he was present for the entire assessment and agrees or modifies the resident treatment and findings.
