Wiki Pacemaker interrogation at time of insertion

Could someone please clarify if interrogation is billable with the insertion or replacement of a pacemaker or defib? My understanding is that it is bundled. Thank you
 
If the physician is inserting a new device or replacing an existing device, for say, end of life -- then interrogation/programming will be included at the time of device placement.
