Can someone please help me? What are the codes for the following procedures?

1)Replacement of a dual pacemaker and implantation of ventricular leads?
2)Inserting leads only?
3)Upgrade from a dual pacemaker to dual ICD?
4)Upgrade from a single ICD to a BiV ICD?
5)Upgrade from a dual to BiV ICD?

Thanks in advance.

1) Replacement of a dual pacemaker would be 33228 however if they implant new leads at the time of the replacement you would code a new system 33206 - 33208 and the removal of the old generator

2) Insert leads without generator insertion 33216 - 1 lead, 33217 - 2 leads (RA and/or RV leads) and 33224 for LV lead

3) Upgrade from dual pacemaker to dual ICD - 33249, 33233 and if DFT testing is done 93641 (26)

4) Single ICD or Dual to Biv - Code 33225 for insertion of LV lead and 33263 or 33264 for the replacement of the generator
