I am billed CPT 33229 and 33222 and 33222 was denied due to bundling with primary procedure. This has been paid by other payers but this particular payer is not and may be one of their guidelines, even though they say the follow CMS guidelines. The pocket was revised/moved due to butting against the left shoulder.

Any suggestions?
