A patient had a knee xray for "pain/trauma s/p TKR"
my supervisor said that i need to assign V54.81 719.46 and V43.65
But I think that for a diagnostic services the first listed s/b 719.46
Can someone clarify it for me

Sounds like your documentation reflects an injury to a knee that had a previous TKR. So the injury is the first dx code, 959.7. Unless you have documentation showing that aftercare specifically directed to the joint replacement was provided, V54.81 doesn't work. Coding the joint replacement status is appropriate.
