Wiki Pain Management bill with J7321 modifier issues

Currently we got claim denied reimbursement for J7321 from Caloptima due to "The procedure code is inconsistent with the modifier used or a required modifier is missing " SINCE Claim billed with Q9966, J7321-RT, 77002-26, 77002-TC, 20610-AG.
If anyone know? please, help. Thanks.

Is this a single use vial? I would look into JZ/JW modifier requirements.

77002 doesn't need modifiers if Professional and Technical components performed on same day by same physician.

You would not append a laterality modifier to the J code.
You indicated: "J7321-RT"
