I read somewhere effect 3/1/2107, these codes cannot be billed greater than 3 times in a 90-day period for same anatomic site, without medical necessity. Anyone else hear of this and if so, is it more likely an individual insurance coverage change and not a national CPC code change? Thanks.

Do you know where you read that? I have a current claim that was denied for reason CO-50 by Medicare for not being deemed a necessity.
