Fellow coders:

Provider lists neutropenic fever, anemia, and thrombocytopenia in his assessment list. Do I use the code for pancytopenia since all three are listed, or must "pancytopenia" also be documented by the provider in order to use it?

Patient is also undergoing chemotherapy but provider does not document a correlation there so I'm not using "drug-induced pancytopenia."

Hi Brenda,
As you may already know, you really need to query/communicate with the provider to have him or her specify the anemia diagnosis. Including any relation to the chemo. Besides that, if it’s not documented I wouldn’t code it.
