Can a physician just say a Pap smear was done or do they have to give a little detail as to how it was done?

We have one physician who sometimes says "Pap smear obtained" and then other times he says;
PAP, Thin prep

Collection Technique: Broom, Brush

Is the physician required to give the information about source and collection or is he able to just say "Pap Obtained"? We were under the impression that he needs to provide a little bit of information to support what he did.

Any thoughts?
