If a pt has had a bx and it came back malignant and needed re-excision; Excision was done and path was sent which came back benign because excision margins were clear. Do you charge "excision" path with malignant dx or path with benign dx?

Use the malignant dx - the reason you had to go back and (re-)excise with wide margins....even if they came back clear/benign.
