Can anyone refer me to guidelines coding surgery encounters. Can you code the encounter without the path report?

You need pathology report to have final CPT for excision between benign or malignant if they do histopath. You have to wait for the result of pathology

Thank you for your feedback. Does this include all encounters (colonoscopy, EGD, lap chole. etc) or just for skin excisions. Thank you.
 
No it is just skin lesions, or any other procedure that contains the qualifier , "benign" or "malignant". For any procedures that do not distinguish you May code the information known at the time of coding or the definitive result if you waited for it. Screening procedures must have the screening V code fist-listed even if there are definitive findings.
