Good Morning! Lately I have been having problems with my pathologist getting paid from several insurances. This last one is from Humana. They are denying for place of service being incorrect. My pathologist works out of the lab at the hospital. I am told that because he works out of the lab only the lab services will be paid. Then they turn around and tell me to rebill with documentation. I am billing with modifier -26, POS 22 and the CLIA # is on the bill. Any one else having this problem or is it just me? Help!
