the pathologist I bill for is kind of rare. he is contracted w/ the hospital, but runs a small o/p pathology office through the hospital using pos 11 in addition to his other account which uses pos 21 and 22.
for the small o/p account I have always billed each individual cpt for the tc and prof. serv. I use mods like TC and TCQ6; for the prof I use 26AQ or 26AQQ6; this account has been billed this way for years
all the sudden I am getting recouped out the ying yang by medicare and its all for the TC part of the services! the new cpt books for 2014 just arrived and I wasn't aware of any changes to the way medicare is paying tc for pathology and google doesn't help

PLEASE HELLLP!!!
