I have a physician that recently started doing patient home visits. I keep getting rejected by Medicare A7-invalid information. 500-Entity's postal/zip code. I have tried several different zip codes for the the Facility "Patients Home". Can anyone please help with this issue. Thank you for any assistance you can provide.
