Our patients complete a patient intake sheet which the provider references in his note as being reviewed. I'm looking for an opinion as to whether that document needs to be signed and dated by the provider. The office note is electronically signed by the provider - will that count as signature for the intake sheet since it is referenced in his note as reviewed by him that date?

Thanks for any opinions!
 
Health Questionnaire Forms

We went back and forth about this with are providers and have come to the conclusion that as long as they reference it, we use it. There is no rules that I've found that it has to be signed.
