Wiki Patient leaving AMA then readmitted

Patient left AMA from the intensive care unit in the morning. The doctor dictated the discharge summary. Later that same evening (same facility) the patient came back as the symptoms got worse and the same doctor readmitted the patient to intensive care. Suggestions on how to code? If so, modifier?
