Is there a CPT code to use when a patient leaves the hospital AMA?
 
You would use the Hospital Discharge Services codes...just make sure the patient actually saw the physician and it wasn't a LWBS...the level of the code would be based on what was done before the patient decided to leave...did they see the physician but not stay long enough for labs....things like that....
