We are a retina specialist. Patient came in, was dilated twice, history was taken by tech.
Tech also did visual acuity. However, doctor never saw patient. There is no documentation in the chart as to why patient left. Does anyone know where information is available that I can show the techs that it is important to document the reason the patient left? I have tried CMS Program Integrity Manual and my malpractice risk management site.
