I have a mom complaining that our office charged 94640. She says they brought in their own nebulizer. It doesn't state in the notes that the treatment was given with the patient's own neb, but if indeed it was, are we in error for reporting 94640?

The 94640 isn't a code to bill for the svn machine equipment itself. If it we would be getting reimbursed $100, instead of $20. I would absolutely bill it those take a good 10 minutes or more of exam room/nurse supervision time. These days in pediatrics we have to fight for every penny to stay afloat.
