I work for an Ophthalmologist and am billing 99426 and 99427 from a 3rd party vendor that manages the care codes. My concern is that I'm seeing the following documentation and I'm just not clear if what they are sending over for me to bill is correct. I see the following billing info and I don't like seeing text and LVM that are unanswered by the patient. Here is how the following comes over.
DOS 10.02.2023 total time 33 minutes.
4min test message no communication with patient.
4 minutes for documentation
25 minutes talking with the patient.
I'm uncomfortable billing the 4-minute text because of no communication with the patient. Without it they would not be able to submit 99426 to our office.
DOS 10.25.23 total time 60 minutes
5-minute LVM
4 minutes for documentation
55 minutes talking to the patient.
Help please very concerned. I've looked everywhere but can't find an answer on billing minutes without communication to a patient under CPT PCM codes 99426 and 99427
