New guidelines on PCNL procedures for 2023 instruct the reporting of CPT 50436 or 50437 in addition to the PCNL code when the urologist creates his own access/dilation. Codes 50436-50437 bundle with PNCL codes - what modifier should be used in these scenarios?
 
I just billed one with the XU modifier. The bundling matrix I use says its allowed with modifier (meaning -59 or equivalent) if justified.
 
I use modifier 59/XU for now. Sure Medicare will update their CCI done the line.
