I am currently billing for a PCP that sees patients in her office as well as a nursing home. As of this month, she has completed her Rheumatology degree. If she sees patients as a specialist, in either the nursing home or her office, and is billing the same codes as she has in the past (ex. 99201-99215 and 99304-99310), how will insurance companies, like Medicare, know that her fee schedule should increase? Is that information linked to her NPI once the insurance companies are made aware of her specialty? I appreciate any help...I am confused.

Ins companies won't know unless you tell them. You need to contact insurance and update provider info. for commercial insurance you probably need to renegotiate the contract as well.
