Wiki Ped. Office visit vs. counseling ???

I had a doctor code for an office visit -99214 to discuss a childs condition, ADD. After reading the note discovered that the patient was not even available for the visit. Only a discussion. What would be the appropriate procedure for coding such a visit if there is one.
 
Counseling / Coordination of Care

Look at the guidelines for choosing your E/M level based on counseling/coordination of care.

There must be a medically necessary reason why patient is not able to participate in the visit in order to code this for a visit with family only.
