does any one know if i can code 15570 with a total pneumectomy and lymph sampling.

here is what i have to go on:

a bulky mediastinal pedicle fat pad was dissected free from the pericardium and rotated towards the left hilum area. This required division of the left phrenic nerve. the pedicle flap was secured in multiple areas to cover all the vascular stumps as well as the covering the bronchial stump.

i would not have thought so, but code 15860 is being used for spy imaging.
 
I don't think CPT 15570 goes deep enough. I believe this code is for skin and subcutaneous tissue. I would try 15734 if muscle was used (which is what I'm thinking).

As for the spy imaging, I would make sure the payer agrees with this. Many payers consider this bundled into the cabg. The WPS Medicare medical director told me specifically not to bill for this.
