EXAM: Pelvic venogram

HISTORY: Left leg swelling with DVT and evidence of left common iliac
vein compression by left common iliac artery

MEDICATIONS:
2 mg Versed
50 mg fentanyl

Fluoroscopy time: 0.9 minutes
Dose area product: 15 gray per centimeter squared

DURATION OF SEDATION:
30 minutes

Intravenous conscious sedation was administered by a dedicated
independent observer with continuous hemodynamic and respiratory
monitoring performed, including the use of pulse oximetry.

FINDINGS/TECHNIQUE:

Signed informed consent was obtained from the patient. A time out
procedure was performed.

The patient was placed in the supine position on the stretcher and
the left groin prepped and draped in standard sterile fashion. All
elements of maximal sterile barrier technique were followed including
cap and mask, sterile gown, sterile gloves, large sterile sheet, hand
hygiene and 2% chlorhexidine for cutaneous antisepsis.

10 cc 1 % lidocaine was used for local analgesia. Using ultrasound
guidance, the left common femoral vein was accessed with a 21-gauge
needle followed by wire and micropuncture sheath.

Venography performed through the micropuncture sheath demonstrated
wide patency of the left common iliac vein without cross pelvic
collateral flow.

Needles and wires were removed and sterile dressing applied. The leg
was wrapped with an Ace bandage.

The patient tolerated the procedure well without immediate
complication and left the radiology department in stable condition

IMPRESSION:

Pelvic venogram demonstrating wide patency of the left common iliac
vein without cross pelvic collaterals.
