Does anyone have any guidance on how to code a Percutaneous Bunion Correction with Computer Navigation?

Doctor is saying “the procedure is basically a Percutaneous Bunion Correction with Computer Navigation- the bunion is corrected making small incisions using internal fixation (which can include pins, wires, plates or screws) to straighten the first metatarsal, (I use pins and wires). The Arthrex system is used to confirm proper alignment of the first metatarsal. This removes the bunion without further surgical shaving or sawing. The 2025 CPT Code book states: 28292 Correction, hallux valgus, with bunionectomy, with sesamoidectomy, when performed; with resection of proximal phalanx base, when performed, any method. No where does the code description mention the word Open or Percutaneous-it clearly states “any method”.”

I am thinking unlisted procedure compared to CPT 28292.

No. This is 28306.
CPT Assistant 2024 very, very clearly goes over this and confirms that, if you did not resect the medial eminence, then it is not a bunionectomy (hallux valgus correction) and that minimally invasive osteotomies without excision of the medial eminence are to be coded with the osteotomy codes. In this case, an osteotomy of the 1st metatarsal was performed. That is 28306.
