Hi-

I have had several instances where OP providers will document a px that is currently being treated for a UTI N39.0 as also having a personal hx of UTI's. My reading of the ICD-10 Chapter 21 guidline #4 History of codes. suggest to me that we cannot/should not use these 2 codes together

“Personal History Codes explain a patient’s past medical condition that no longer exist and is not receiving any treatment, but that has the potential of recurrence, and therefore may require continued monitoring”

Can someone please weigh in on this? I have asked numerous people and the answers seem all over the place!
