For the PFSH, is it required to be reviewed on the same date of service? I mean fully documented that it was reviewed at the appt. We have a provider that will click reviewed the next day and I'm not sure that counts for coding. I can't find any documentation regarding this.

Take into consideration that the encounter is for the "face to face" services so, with that being said, anything regarding that visit for that date of service, means it was reviewed "with the patient" and a statement of "reviewed and no changes" or reviewed and patient states sister has DM" or something to that effect should be documented in order to count the pfsh. If the provider actually reviewed this information with the patient then no problem, but if not then maybe some documentation guideline education is needed. You can get this from the 1995 or 1997 guidelines and also CMS website.
