1. Can someone tell me if I can count points towards when physician documents " There are no changes in her past medical, family or social history since her last visit" Do I count this as Complete PFSH for a follow up visit (99211-99215). Also can I use the past history that is mention in HPI.

2. Physical Exam: when physician documents Neck: no goiter does that count as "lymphatic" or do I just give credit for body system
