Does anyone know if the PHE has been extended for telehealth visits?

One of my providers is asking if they can still do a regular office visit by telehealth. Do we still code this is as a 99212-99215 with the modifier 95? Are there certain requirements that have changed now that we're coming out of the pandemic?
 
Unfortunately, just like when telehealth ramped up, it is all payor specific. Medicare has stated they will cover telehealth codes on the temporary list until at least the end of 2023.
Some want -95, some want -GT. Some want POS 02. Some want whatever the POS would have been if not for the PHE.
