Phlebotomy bundled with draw fee

When a patient has labs drawn, we bill 36415 and lab test (like, CBC). Sometimes the patient will need a phlebotomy the same day. Is the draw fee still payable, once a phlebotomy is billed? (Many insurance plans have bundled these together.) If it is payable, should I add a 59 modifier to the draw fee?

It really depends on the payer. We go ahead and bill both, there is no conflict between the two. It is not necessary to add -59, if anything a -51 could be added for some payers.
 
How do you handle an E/M and phlebotomy on the same day. Can you bill with no modifier or bill with Mod-57?
