can you help me to find out if PDT for extremities approved by FDA ?

It is approved for Actinic Keratosis icd-9 702.0 As long as it is AK's - location of arms, face, scalp is approved. For Medicare in Texas I have no problem getting paid. If it is a commercial carrier - sometimes precert is needed.

DUSA pharmaceuticals has a precert dept and they do alot of our precerts for us.
