One of our family physicians has recently found a patient that came in for ONLY a physical. Our physician decided to x-ray her chest after hearing some wheezing. He found a mass on a lung. He also diagnosed thrush for this patient. He is trying to bill for the physical and a 99214. Am I wrong telling him he doesn't have enough documentation to support the 99214? He found the lump during the physical exam. He's telling me that the lump was an incidental finding and justifies the 99214 with the physical. Please help!
