i am doing audit for inpatient EM visit

Initial visit documentation states that PHYSICAL EXAM is unable to perform since patient was not co-operating [ let me tell you this patient has psychological disorder ] can we consider comprehensive physical exam??

I know that history can be considered comprehensive if patient is unable to give it to physician due to medical condition...but i am not sure about physical examination

need help.
