Is it necessary for a C.C. when doing a physical exam. I am under the assumption yes for every visit, right or wrong give me some guidance.

Think of your CC has the reason for the visit. How do you determine whether or not the exam is prevenative (physical) or not without it? I wouldn't say the HPI is required but some sort of statement is necessary to establish a reason for the visit.
