Physical status modifiers

HI! Fairly new to anesthesia coding and was looking for a little clarification on dxs for Physical Status Modifiers. To bill for the P3-P5 does the dx have to be different from the primary dx (an underlying condition)? I was trained that is does, but wanting to make sure that's correct?!! TIA!
 
The first-listed diagnosis should always be the reason for the surgery. It is my understanding that it is not required to have the diagnosis(es) that supports the physical status modifiers, however, I do use them as additional diagnoses when space allows (only 4 ICD-10-CM codes per CPT code.)
