Does anyone have any good sources for documentation requirements for physical therapy? I am looking for something I can show my physical therapists that very clearly states what they need to document on their therapy notes in order to justify their billing. I have googled like a mad woman and can't find anything that is direct and to the point. Thank you!!
 

I would have them follow the S.O.A.P type of documentation. The therapists that I work for also have flow sheets that they document all of the exercises they do in a particular session in addition to their daily note. We treat a lot of Scoliosis patients with the Schroth method, so there is a Schroth flow sheet. Our other Orthopedic patients have their own flow sheet for the exercises preformed on a particular day. Neurological patients....they too have their own flow sheet.
