Wiki Physical therapy evaluation

Hello there!
I have a physical therapist who was not able to complete the initial evaluation on the 1st visit due to patient being late and therefore the PT was only able to complete the subjective part of the evaluation. Patient returned on a later date to complete the evaluation. How should I code the visits - just the evaluation code on the 2nd visit only? Is there an appropriate code for the 1st visit?

We cannot do anything for the 1st visit as the evaluation is not completed.


You are right on coding the evaluation code only on the 2nd visit.

Our practice is adding a physical therapy department to our practice, my physician wants the therapists to do prehab and rehab with the surgical patients at the ambulatory surgery center on the day
of surgery. How do I obtain authorization for this type of treatment or code for these services? would this be similar to an evaluation and rehab visit?
