If a patient is being seen for PT and has not had surgery or a repaired fracture, is it going to be an initial, subsequent, or Sequela encounter?

Also if a patient is able to self refer how is that coded?
 
My understanding is that when PT see's the patient it is for treatment for aftercare for routine healing of fracture so we have told our Therapists to use D-subsequent. If a patient self refers you would use A for initial encounter. We don't take self referral patients but the webinar I watched said A.
