Wiki Physician Assistant during procedure

Can anyone help me find definitive documentation on whether or not the physician must dictate what the PA performed during the procedure?

I have checked on CMS, but am unable to find any concrete information. I have also checked two other sites and am getting conflicting information.

I realize that I should know this... but I just keep getting conflicting information, so nerve racking!

It's always been my understanding that each physician dictate what they did and only what they did. I work for a payer and that's what we expect.
