My physician wants to charge for filling out CMS form 484 - Certificate of Medical Necessity for Oxygen. Is there a code for this?

It's been a long time since I have worked in DME but the only code that I know of for filling out CMN's is for power wheelchairs. The oxygen CMN is sent to the provider by the DME company and its my belief that as the prescribing doctor he is required to provide the medical necessity to Medicare. It's his duty.The patient is usually not present when the CMN is filled out. If I am wrong I am sure someone will let me know :)
