Can a non-physician practioner, who is in the exam room with the patient and physician, dictate the visit on behalf of the physician?

A PA/NP/CNS is allowed to act as a scribe for a physician but there are rules governing this. You can research on this site and the internet "scribe" or "PA acting as scribe" and you can see the rules and regs for this.
 
Thank you for the suggestions. If the office is not utilizing EHR at this time, and the Physician Assistant is dictating from visually seeing and hearing what was performed by the doctor, would that be allowed?
