When billing home visits by a physican how is medical necessity being determined? The guidelines state that patient does not need to be home bound but wants medical necessity to be determined. This is a gray area to me. Chronic DM or CHF is medical necessity but the patient may be stable and not house bound. The guidelines do not state what constitutes medical necessity and there is no LMRP anymore to give specifics. Any ideas?
